Our Story

In 2021, we hit the reset button

We founded Continuum Therapy Partners after spending decades in the contract rehab industry—as therapists, executives, and operators. We’ve worked for small companies. Large companies. Companies owned by people we knew personally, and companies owned by private equity firms we’d never met. All of them had one thing in common: they were built to be sold.

We watched that model create compliance risks. We watched it burn out great clinicians with unrealistic productivity demands. We watched companies cut wages across the board during a global pandemic—and then wonder why turnover spiked and morale collapsed.

Every decision ran through the lens of contract margins and EBITDA, because the real client was the next private equity buyer—not the customers, their residents, or the clinicians providing care in their facilities.

We built Continuum to support the therapists and facilities we serve, not a future buyer. That means no restrictive covenants designed to trap clients or restrict the employment of therapy professionals without their knowledge. No feeding off the bottom line of nursing home operators. Instead, we offer transparent, strategic, authentic partnerships. We earn our business every day.

What We Believe

Three commitments

We put caregivers first

A culture built around therapists — not a spreadsheet.

We invest in growth

Ongoing education, mentorship, and skill development, because well-supported therapists deliver better care.

We hold a high bar, together

Great outcomes take all of us showing up fully. We ask a lot of our team — and we back that up with real support in return.

The Result

Real value for real stakeholders

Higher quality outcomes

Lower staff turnover

Increased reimbursement accuracy

Lower therapy cost

We started with 9 skilled nursing facilities in North Carolina. Today we partner with hundreds of facilities across 17 states. We still own the company — no private equity — and we intend to keep it that way.

Our Culture

What it feels like to wear the teal

Culture isn't a poster in the break room. It's how decisions get made, how people get heard, and how it feels to come to work.

You are heard

Partner panels, the Path Award, Continuum Central, and direct access to management — real avenues for your voice, not a suggestion box.

Partners, not just employees

Day-one benefits at every tier — even PRN — because commitment is supposed to run both directions.

Clinical autonomy

Therapists shape their own treatment plans for the person in front of them. That creativity improves outcomes — and keeps the work meaningful.

Rooted in community

Advocacy, outreach, and education — for health care providers and the communities we serve — are part of the job description, not an afterthought.

The people behind the pledge
